What the form measures

The form is built on the four levels of training evaluation that most L&D teams use, often called the Kirkpatrick model:

LevelQuestionWhere in the form
1. ReactionDid people find it relevant and well run?Part A: seven ratings from 1 to 5
2. LearningDid knowledge and confidence go up?Part B: before and after self-ratings
3. BehaviourWill they use it at work?Part C: what they will apply, and blockers
4. ResultsDid it change a business outcome?Follow-up section, 60–90 days later

How to use it

  1. Hand out or send the form in the last ten minutes of the session, not afterwards by email.
  2. Keep the department field optional so people answer honestly.
  3. Average the Part A ratings per item. Anything below 3.5 needs attention.
  4. Compare before and after scores in Part B for each person, then the average change.
  5. Share Part C answers with managers so they can support people in applying what they learned.
  6. Complete the follow-up section after 60–90 days using observation and business data.

Better than a smile sheet

Many training feedback forms stop at "did you enjoy it?". That tells you little about value. Self-ratings of learning are useful, but a short test is better evidence. Pair the form with a quiz at the end of the training, and compare test results with the ratings.

Questions to add for specific training

  • Compliance training: "I know what to do if I see a breach of this policy."
  • Software training: "I can complete [task] in the system without help."
  • Sales training: "I will use the new pitch in my next five calls."
  • Online training: "The platform was easy to use on my device."

Evaluating training at scale

With many sessions a month, paper forms become a data-entry job. On AcadifyLab you can end each course with an online test to measure learning, see completion and test reports on every plan, and from the second plan up get an AI summary of learner feedback. See employee training software.

FAQ

Common questions

Is this training evaluation form free?

Yes. The Word file is free to download, edit and share.

What is the Kirkpatrick model?

A widely used way to evaluate training at four levels: reaction, learning, behaviour and results. This form covers the first three at the end of training and the fourth in a follow-up section.

When should training be evaluated?

Collect reaction and learning at the end of the session, then follow up on behaviour and results 60–90 days later.

How is this different from a student feedback form?

The student feedback form rates a teacher or course. This form is for workplace training and also asks how people will apply what they learned.
