Questions in the form

Students rate ten statements from 1 (strongly disagree) to 5 (strongly agree):

  1. The teacher explained topics clearly.
  2. The pace of the class was right for me.
  3. I could ask questions and got useful answers.
  4. Examples and practice questions helped me learn.
  5. Study material and notes were useful.
  6. Tests and homework matched what was taught.
  7. Classes started and ended on time.
  8. The online or classroom setup worked well.
  9. I feel more confident in this subject than when I started.
  10. I would recommend this course to a friend.

Then four open questions: what they liked most, what the teacher should do differently, which topic is still difficult, and anything else.

How to get honest answers

  • Keep it anonymous. The name field is optional for a reason. Students are more honest when they cannot be identified.
  • Explain why you are asking and what you did with the last round of feedback.
  • Give class time to fill it in. Forms taken home come back half-empty.
  • Keep it short. One page gets completed; three pages get skimmed.
  • Have someone else collect it where possible, not the teacher being rated.

How to read the results

Average each rating question across the class. Anything below 3.5 is worth a look. Then read the open answers and group them into themes, such as pace, notes or doubt clearing. Pick one or two changes, tell the class what you are changing, and check again next time. Feedback that visibly leads to change gets better responses in future.

Collecting feedback online

To run this form online, copy the questions into Google Forms or Microsoft Forms. If you teach on AcadifyLab, feedback from learners can be summarised with AI from the second plan up, so you see the main themes without reading every response. See AI features.

FAQ

Common questions

Is this feedback form free?

Yes. The Word file is free to download, edit and use in your school, institute or training programme.

Should student feedback be anonymous?

Usually yes. Anonymous forms get more honest answers. Keep the name optional, as this form does.

How many questions should a feedback form have?

Ten to fifteen. This form has ten rating questions and four open questions, which fits on one page and takes about five minutes.

Can I use it for online courses?

Yes. Copy the questions into Google Forms or Microsoft Forms, or into your course platform's survey tool.

How is this different from a training evaluation form?

This form is for students rating a teacher or course. The training evaluation form is for workplace training and also asks how learning will be applied on the job.
